A doença pulmonar obstrutiva crónica (DPOC) caracteriza-se por limitação persistente do fluxo aéreo, dispneia progressiva e redução da capacidade funcional, condicionando impacto significativo na qualidade de vida. A fraqueza dos músculos inspiratórios constitui um fator relevante na intolerância ao esforço e no agravamento da sintomatologia, e o treino dos músculos inspiratórios (TMI) tem sido proposto como intervenção complementar na reabilitação pulmonar.
Objetivo: Mapear e analisar a evidência científica existente sobre os efeitos do treino dos músculos inspiratórios na reabilitação de pessoas com DPOC.
Métodos: Realizou-se uma revisão de escopo, com pesquisa estruturada em bases de dados científicas, que incluiu estudos sobre a eficácia do TMI em doentes com DPOC. Foram considerados estudos que avaliassem resultados como força muscular inspiratória, dispneia, capacidade funcional, tolerância ao exercício e qualidade de vida relacionada com a saúde.
Resultados: Os estudos analisados demonstram resultados consistentes na força dos músculos inspiratórios, particularmente em indivíduos com fraqueza inspiratória documentada, constatando-se ainda a melhoria da dispneia, embora com magnitude variável. Os efeitos na capacidade funcional e na tolerância ao exercício mostraram-se mais evidentes em subgrupos específicos ou quando o TMI foi aplicado de forma isolada. A qualidade de vida apresentou melhorias, dependentes do contexto de aplicação, e, quando integrado em programas de reabilitação pulmonar, o TMI revelou utilidade como intervenção adjuvante, ainda que os resultados adicionais nem sempre sejam uniformes. A individualização da prescrição e a progressão adequada da carga emergem como fatores determinantes para a eficácia.
Conclusão: O TMI constitui uma intervenção segura e potencialmente benéfica na reabilitação de pessoas com DPOC, sobretudo na presença de fraqueza inspiratória. A sua implementação deve ser individualizada, integrada em programas estruturados de reabilitação pulmonar e sustentada na prática baseada na evidência, assumindo a enfermagem de reabilitação um papel central na sua operacionalização.

Inspiratory muscle weakness is a relevant factor in exercise intolerance and the worsening of symptoms, and inspiratory muscle training (IMT) has been proposed as a complementary intervention in pulmonary rehabilitation. Aim: To map and analyze the existing scientific evidence on the effects of inspiratory muscle training in the rehabilitation of patients with COPD. Methods: A scoping review was conducted, with a structured search in scientific databases, including studies on the effectiveness of inspiratory muscle training (IMT) in patients with COPD. Studies assessing outcomes such as inspiratory muscle strength, dyspnea, functional capacity, exercise tolerance, and health-related quality of life were considered. Results: The studies analyzed demonstrate consistent results regarding inspiratory muscle strength—particularly in individuals with documented inspiratory weakness—and also show improvements in dyspnea, albeit of varying magnitude. Effects on functional capacity and exercise tolerance were more evident in specific subgroups or when inspiratory muscle training (IMT) was administered in isolation. Quality of life improved depending on the context of application; furthermore, when integrated into pulmonary rehabilitation programs, IMT proved useful as an adjunctive intervention, although the additional benefits were not always uniform. Individualized prescription and appropriate load progression emerge as key factors for efficacy. Conclusion: Inspiratory muscle training is a safe and potentially effective intervention in the rehabilitation of patients with COPD, particularly in the presence of inspiratory muscle weakness. Its implementation should be tailored to the individual, integrated within structured pulmonary rehabilitation programs, and grounded in evidence-based practice. Rehabilitation nursing plays a central role in its delivery and optimization.
